The DS-82 form is used to renew your expired passport, and can only be used if you have your passport, were over 16 years old when your passport was issued, and your name is the same as it appears on your expired passport, or have had your name changed by marriage or court order and can submit proper certified documentation to reflect the name change.

How do I fill out Form DS-82?

Starting at the top of page one, check the box that reflects what type of document you are looking for (passport book, card, or both). Then starting on line one enter your current last name. If your name was changed through marriage or court order you will be able to include this information on line 11 with a certified copy of your marriage license or court order. Still on line one, fill in your first and middle name. Fill in your date of birth on line 2. Note there are two digits for month and day and four digits for the year. Check the appropriate box for line three; M is for male F is for female. Line four asks for your place of birth. Leave a space in between city and state or between city and country. Enter your social security number on line 5 and your email information on line 6. If you choose to provide your email address in Item #6 on this application, Passport Services may use that address to contact you in the event there is a problem with your application or if you need to provide additional information. Fill in your primary contact phone number on line 7. Line 8 includes your full mailing address. You may use a PO Box, company address, permanent address, or you may address it "In Care Of" or "Attention To" and write the recipient's name. Continue with your city, state, zip. If you live outside of the US fill in your country of origin. Line 9 ask that you enter any other last names you have used previously. Line 10 asks you to enter your most recent passport book or card number as well as the date it was issued in its corresponding box. Line 11 is for those who have changed their last name through marriage or court order. Check the correct box regarding why your last name was changed, the place the change took place, and the date the change was made. Staple a color photograph, 2 inches by 2 inches, of you by yourself, taken within the last six months. The image size measured from the bottom of your chin to the top of your head (including hair) should not be less than 1 inch, and not more than 1 3/8 inches. The photograph must be in color, clear, with a full front view of your face. The photograph must be taken with a neutral facial expression or a natural smile and with both eyes open, and printed on photo quality paper with a plain light background. The photograph must be taken in normal street attire, without a hat or head covering, unless a signed statement is submitted by the applicant verifying that the hat or head covering is part of recognized, traditional religious attire that is customarily or required to be worn continuously when in public, or a signed doctor's statement is submitted verifying the item is used daily for medical purposes. Headphones, bluetooth, or similar devices cannot be worn in the photograph. Glasses or other eye-wear are not acceptable unless you provide a signed statement from a doctor explaining why you cannot remove them due to medical reasons. USE CAUTION WHEN STAPLING YOUR PHOTO: Use 4 staples vertically (up and down, north to south) in the corners as close to the outer edge as possible. Do not bend the photo. Sign and enter today’s date on the bottom of page 1. Starting on page two, line one, enter your last name, first name, and middle name. Then enter your date of birth, note there are two digits for month and day and four digits for the year. Starting on line 12, enter your height, then moving along to line 13 enter your hair color and line 14 is for your eye color. Enter your current occupation on line 15 as well as your employer or school information on line 16. Line 17 asks for additional contact numbers. Enter your address information again for line 18. Enter your emergency contact person’s information on line 19, and finally on line 20 fill in any travel arrangements you currently have planned with travel dates and countries to visit. You have now completed the DS-82 form. Mail this form in with all proper attachments and fees.
