About this Form:
Use Form 4868 to apply for 6 more months, or 4 if you are out of the country and a U.S. citizen or resident, to file Form 1040, 1040NR, 1040NR-EZ, 1040-PR, or 1040-SS.
How do I fill out Form 4868?
Part I wants your identification information
Starting on line 1, enter your name and address. If you file a joint return, include both spouses’ names in the order in which they will appear on the return. Enter the address you want correspondence regarding this extension to be sent to you.
On line 2, for those who file a joint return, enter the SSN that you’ll show first on the return. On to line 3, enter the other SSN to be shown on the joint return.
Part 2 needs information regarding your individual income tax
For line 4, enter the total tax liability you expect to report. For line 5, enter the total payments you expect to report. For line 6, subtract line 5 from line 4. If line 5 is more than 4, enter 0.
Line 7 concerns the amount you’re paying. If you find you can’t pay the amount shown on line 6, you can still get the extension. But you should pay as much as you can to limit the amount of interest you’ll owe. Also, you need to be aware that you may be charged a late payment penalty on the unpaid tax from the regular due date of your return.
Line 8 is asking you to check the box if you’re out of the country on the regular due date of your return. Line 9 needs you to check the box if you did not receive wages subject to U.S. income tax withholding and your return is due June 17.
