Form 1040-V is a statement you send with your check or money order for any balance due on the “Amount you owe” line of your Form 1040 or 1040-NR.
How do I fill out Form 1040-V?
The purpose of Form 1040-V is to attach as a payment voucher and mail with the payment you owe to the IRS from Form 1040 or 1040-NR.
Starting with line 1, enter your social security number (SSN). If you are filing a joint return with your spouse or domestic partner, enter their SSN in the space provided on line 2.
Moving over to line 3, enter the amount you are paying in the dollars and cents spaces provided.
Down to line 4, enter your name exactly as shown on your return. Do this for your spouse as well if filing jointly. Then enter your current home address number and street, your apartment or condo number if applicable, and your current town or city, state, and zip code. If you live in a foreign country, use the last lines to enter the country’s name, province or state or country, and the foreign postal code.
You have now completed Form 1040-V.
