Use Form W-9 to provide your correct TIN to the person who is required to file an information return with the IRS to report.How do I fill out Form W-9?
Starting on line 1, enter the name shown on your most recent income tax return. Moving to line 2, enter your business name as it is shown on your Statement of Information. Line 3 requires you check the correct box for your business’ designation, such as a sole proprietor, LLC, or corporation. Only fill out line 4 if you have exemptions, otherwise leave this section blank. Moving to line 5, enter your business’ street address, and on line 6 enter the city, state, and zip code. You will notice to the right there is a space to add a requester’s name and address. This is an optional field and can be left blank. Line 7 is also optional. This line asks that you list any account number associated with the business entity. Part 1 needs your social security number if you are an individual sole-proprietor, but will need your employer identification number if you are any other type of business. Part 2 requires your signature to certify that the number shown on this form is your correct taxpayer identification number (or that you are waiting for a number to be issued to me); and you are not subject to backup withholding because: (a) you are exempt from backup withholding, or (b) You have not been notified by the Internal Revenue Service (IRS) that you are subject to backup withholding as a result of a failure to report all interest or dividends, or (c) the IRS has notified you that you are no longer subject to backup withholding; and You are a U.S. citizen or other U.S. person; and the FATCA code(s) entered on this form (if any) indicating that you are exempt from FATCA reporting is correct.
