About this Form:

Form W‐4P is for U.S. citizens, resident aliens, or their estates who are recipients of pensions, annuities, and certain other deferred compensation to tell payers the correct amount of federal income tax to withhold from your payment(s). You also may use Form W‐4P to choose not to have any federal income tax withheld from the payment or to have an additional amount of tax withheld.

How do I fill out Form W4-P?

The purpose of Form W-4P is to let the payers of your pensions, annuities, and certain other deferred compensation know the correct amount of federal income tax to withhold.

Starting at the top, fill in your first name and middle initial. Moving to the next box, enter your last name. Moving to the right, enter your social security number. On the next line down, enter your home address number and street. If you live on a rural route, enter that information in the field provided. Below this, enter your city or town, state, and zip code.

Moving down, check the box on line 1 if you do not want any federal income tax withheld from your pension or annuity. For line 2, write in the number of allowances or enter a dollar value in the space provided and also check the box to indicate your marital status. For line 3, you can choose to withhold additional funds from your payments. Enter any additional amounts that you want withheld from each pension or annuity payment.

Finally, sign and date the form with today’s date. Return this form to the payer of the pension or annuity. You have now completed Form W-4P.
