This form is used to apply for a new passport. After you complete this form, you must submit this application in person to a designated acceptance agent.How do I fill out Form DS-11?
Starting at the top of page one, check the box that reflects what type of document you are looking for (passport book, card, both). Beginning on line one, fill in your last, first, and middle name. Fill in your date of birth in line 2. Note there are two digits for month and day and four digits for the year. Check the appropriate box for line three; M is for male F is for female. Line four asks for your place of birth. Leave a space in between city and state or between city and country. Enter you social security number on line 5 and your email information on line 6. If you choose to provide your email address in Item #6 on this application, Passport Services may use that address to contact you in the event there is a problem with your application or if you need to provide additional information. Fill in your primary contact phone number on line 7. Line 8 includes your full mailing address. You may use a PO Box, company address, permanent address, or you may address it "In Care Of" or "Attention To" and write the receiver's name. Continue with your city, state, zip, and if outside of the US fill in your country of origin. Line 9 ask that you enter any other last names you have used previously. Stop here and move on to page 2. You must wait to fill out the rest of page one when you are in-person with an authorized agent. Starting on page two, line one, enter your last name, first name, and middle name. Then enter your date of birth, note there are two digits for month and day and four digits for the year. Line 10 is asking for your parental Information. Enter Mother/Father/Parent’s first and middle name, and then enter your Mother/Father/Parent’s last name in this section. Note that this forms wants your parent’s last name at their birth. For instance, enter your mother’s maiden name if she took your father’s last name when they married. Moving down, enter the parent’s date of birth: there are two digits for month and day and four digits for the year. Next, enter the parent’s place of birth. On the right check the correct box if they are male or female, and if they are a US citizen. Moving down, enter the other parent’s information. Just like before, enter Mother/Father/Parent’s first and middle name, and then enter your Mother/Father/Parent’s last name. Moving down, enter the parent’s date of birth, and then enter the parent’s place of birth. On the right check the correct box if they are male or female, and if they are a US citizen. Line 11 wants to know if you have been married before. If yes, continue to fill out this section. FIll in your current spouse or most recent spouse’ full name. Continue with their date of birth: two digits for month and day and four digits for the year, and their place of birth. Moving down, check the correct box answering if your current or most recent spouse is a US citizen. Next, enter the date of the marriage: two digits for month and day and four digits for the year. Next, check the correct box indicating if you are widowed or divorced. If yes, enter the date. If not, move onto line 12. Enter your additional contact number if you have one, and indicate if it is your home, cell, work, or fill in the blank if another type of number. Fill in your occupation on line 13, and your employer or school name on line 14. Moving down to line 15, enter your height. On line 16 fill in your hair color, and on line 17 fill in your eye color. If you know your travel plans, enter the departure date on line 18 followed by the return date, and the country(ies) you plan to visit. Down to line 19, enter your permanent address if you listed a P.O. Box under Mailing Address or if the residence is different from Mailing Address. You cannot use a PO Box as a permanent address. Onto line 20, you need to fill in your emergency contact’s information. Provide their full name, address, city, state, zip code, contact phone number, and your relationship to the emergency contact (spouse, parent, etc.). Line 21 wants to know if you have applied for or been issued a U.S. Passport Book or Passport Card? If not, you have completed this form. If you have applied for been issued a passport book or card, you need to continue on line 21 and fill in your name as printed on your most recent passport book, most recent passport book number, most recent passport book issue date, and check the correct box regarding status of your most recent passport card: Submitting with application, Stolen, Lost, In my possession if expired.
